- What to Expect (Job Responsibilities):
- Conduct outbound calls to newly enrolled VA Medicaid members to establish first contact and complete health screenings
- Assist members in navigating and maximizing their health plan benefits, including scheduling appointments and arranging transportation
- Provide outreach to locate and support disconnected members with special needs
- Conduct research to contact members that Molina Healthcare has been unable to reach
- Participate in ongoing or project-based activities requiring extensive member outreach
- What is Required (Qualifications):
- High School Diploma or GED
- Minimum 1 year of experience working with underserved or special needs populations
- Must have a valid driver's license with a good driving record and reliable transportation
- Experience in a healthcare-related field is a plus
- Ability to collaborate effectively with a multidisciplinary team
- How to Stand Out (Preferred Qualifications):
- Associate's Degree in a healthcare-related field (e.g., nutrition, counseling, social work)
- Bilingual skills based on community needs
- Familiarity with healthcare systems and community resources
- Knowledge of health education and social services
- Current Community Health Worker (CHW) Certification preferred
