Join the Federal Government's Telehealth Revolution: Make a Difference in Patient Care The Federal government of the United States is seeking an experienced Registered Nurse to join its team as a Telehealth Nurse, providing remote care coordination and patient support to high-risk patients with chronic conditions. As a Telehealth Nurse, you will play a vital role in reducing unscheduled ER visits, unscheduled walk-in clinic visits, and bed days of care by teaching patients the self-management skills needed to stay well. About the Role The Home Telehealth nurse is a professional nurse who monitors patient status and coordinates care across the healthcare continuum. Key responsibilities include: Coordination of care and disease management education Medication management and reinforcement of the clinical treatment plan Providing patient support and teaching self-management skills Reducing unscheduled ER visits, unscheduled walk-in clinic visits, and bed days of care Collaborating with healthcare professionals to ensure comprehensive care Essential Qualifications To be eligible for this role, you must meet the following essential qualifications: United States Citizenship Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies: The Accreditation Commission for Education in Nursing (ACEN) The Commission on Collegiate Nursing Education (CCNE) Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia Proficient in written and spoken English Ability to pass pre-employment physical examination Participation in the seasonal influenza vaccination program and COVID-19 vaccination program Preferred Qualifications The following qualifications are preferred but not required: Two or more years of Case Management or Care Coordination experience Prior Home Telehealth experience Two or more years of Medical-Surgical experience Bachelor's degree in Nursing or related field What We Offer As a Telehealth Nurse with the Federal government, you will enjoy a comprehensive total rewards package, including: Competitive salary ($72,870 - $142,731 per year) Regular salary increases and potential for performance awards 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA Federal health/vision/dental/term life/long-term care insurance Opportunities for career growth and professional development Work Environment and Company Culture As a Telehealth Nurse, you will work in a remote setting, with the flexibility to work from home or other remote locations.
