As a Field Case Management Coordinator, you will play a vital role in ensuring that our members receive the best possible care and resources to manage their health and well-being. Conduct comprehensive assessments of members' health needs and develop individualized care plans to meet their specific needs and goals. Collaborate with healthcare providers, community resources, and other stakeholders to coordinate and facilitate the delivery of quality healthcare services for members. Monitor and evaluate the effectiveness of care plans, making necessary adjustments and referrals as needed. Serve as a liaison between members, healthcare providers, and insurance companies to ensure seamless and efficient communication. Educate members on their benefits and resources available to them, as well as promote self-management of their health conditions. Document all interactions and interventions with members and maintain accurate and up-to-date case files. Identify and address barriers to care, such as social determinants of health, and connect members with appropriate resources to overcome these barriers. Collaborate with interdisciplinary teams to identify and implement best practices for case management and improve overall quality of care. Stay current with industry trends and best practices in case management to continuously improve the services provided to members.
